Alexandria Orthodontics Showcase: Severe Crowding and Underbite with Lower Canine Extractions

In this week’s showcase, we have a special young lady who had been told in every consult that a surgery option was her only option. Dentally, she presented with a full class III underbite with severe upper and lower crowding. Skeletally, she had a prognathic lower jaw. Besides the concern of having a lower lip protrusion, the patient had no other concerns about her profile.
Though a surgical option was one of the presented options, the nonsurgical option would include braces with the extraction of the lower canines for a class III camouflage of her mandibular prognathism. Already missing upper first molars, we would use a Nance appliance to retract upper premolars to assist in the resolution of the severe anterior crowding.
While taking advantage of accelerated tooth movement from the extractions and Propel treatment, much of the transformation occurred in the first 9 months. The most challenging aspect of any treatment is finishing the treatment with a good occlusion and acceptable torque. This case was no exception. All together, the treatment time totaled 20 months. Upper and lower fixed retainers from canine to canine was an important part of her retention plan to maintain the upper anterior torque. At the conclusion of her treatment, the patient was able to receive the smile of her dreams without surgery in under two years.
