Trial nonextraction for severe crowding⁠

As a rough estimate, severe crowding can be described as a degree to which a tooth (or more) has been completely blocked out. Crowding without bite concerns is generally thought of as soft tissue pressure against the teeth. In some cases, extractions are an option offering a resolution of crowding and a stable result. However, lip posture and overall facial aesthetics must be considered during a comprehensive orthodontic exam. ⁠

In our showcase of the week, our patient presented with severe crowding leading to traumatic crossbites. Traumatic occlusions have lead to recession and enamel wear. Soft tissue forces had lead to retroclination of the incisors contributing to the crowding. A combination of interproximal reduction and proclination while watching for progressing gingival recession provided enough space to result the crowding. After 18 months of braces treatment, the patient achieved the smile of his dreams. ⁠
