So... This happens when you play Mario Kart 7 without a Mii

Normally this is impossible, as when setting up your 3DS system, you are forced to create your own personal Mii in the Mii Maker. However, I'm using the 3DS emulator, Citra, which can boot up the game without the menu, hence, the game fails to register a Mii, and... this happens.

The same goes for Tomodachi Life, and surely any 3DS game that uses Mii.

If anyone wants to know why this happens, the actual model of the Miis in this game is just the suit itself (with some extruded polygons for the neck), the game pulls the Mii head data from the 3DS itself and renders it in-game in real time on top of the suit. If there is no Mii, the game just loads a placeholder square "face" texture were the Mii head should usually go.

It should say something like "You don't have any Miis saved on the console" or give you a "guest" mii like Wii games did

I think that's because on a normal 3DS the user is forced to have at least one Mii (the main one linked to your account), so by Nintendo's perspective it wouldn't make sense to add a real placeholder since at least one will always be there.

A bit of a fact.
Not only 3DS does that when you don't have Mii data. The Wii, Wii U, and Switch does that as well. It's because all Mii data are stored in the NAND and on extra data or extdata.

Hence, trying to play a Mii game without the data will result a headless Mii with triangle neck or a floating square polygon. However, there's an exception where they store Mii data in the game's data such as the defunct Miitomo for mobile (tho only display blank Mii head and a square model for the face)

I did the same thing on Citra a couple weeks ago, but my Mii didn't look exactly like that... the flat square bore a barred red circle, making it look even more weird and unsettling.

I am gussing the quare head mii was used for testing the online lobby and has no back side since it was never meant to be used in gameplay
