I was watching Stud Pack channel where they were building a carriage house (garage with living space above) and the inspector told them it was code to have the foam board sandwiched in between the wood pieces. That makes more sense than having it on only one side. Although, I don't know if faced foam board would work.

Not really very effective unless you have a proper thermal break - which you do not. The plates of the header are still a significant thermal transfer. Also, suppose you didn't use the header insulation? You could still have a sealed dead air space - which is what you are trying to create with insulation in the first place. The rigid insulation only facilitates the thermal transfer across your otherwise sealed dead air space. If you are worried about vapor getting into that dead air space, you can put a vapor barrier over it. I would like to see thermometer readings and inferred images of the Insulated Header shown vs one next to it with a sealed dead air space header.

It is not code. You have solid king header for exterior wall.
