The golden star is actually kind of a meme in Russia. I remember as a kid seeing a lot of people having and using them. It was like oh, you have a bruise, or maybe your skin is irritated, or whatever else, use a star it'll get better.

My favorite 'realistic' fact in game is that your pmc cant run for more than 10 seconds and is terrible with the guns they should be trained for but can complete a surgery on themselves and only themselves in 10 seconds with a 100% success rate so your character is more like a world record master surgeon than a pmc

Can you imagine the turtle shell meta? You barter for the class six armored rigs and fill your packs with them, and the second you hear gunshots you anchor down, turn around, and duck your head, while your buddy tries to find the shooter.

Instead of having weapon durability burn so fast, maybe they could have a "cleanliness" stat. It could basically operate the exact same as durability with the same burn rate but feel more realistic

it's really easy to understand tarkov's "realism":

substitute "realistic" with "frustrating";
done, everything now completely makes sense.

Don't forget about the mysterious unidentified scav boss running to every single container stealing all the scope mounts from their respective scopes but leaving the actual scope behind. Kinda new to the game but it really sucks finding a good scope that's missing its mount that I can't even buy a mount for yet.
