When going for ice water immersion or cold shower, how long should the cold therapy last?

Excellent educational video, thank you! I do have a question that has caused me a bit of confusion. I've heard some people say that the swelling and inflammation after an acute injury (like a sprained ankle) is the body's natural response to the injury, and the increased blood flow to the injured area helps to bring increased nutrients, remove waste products, and brings the necessary materials needed to begin healing. Proponents of this line of thinking advise against immediately applying ice. I've heard similar advice when it comes to fevers. They say if you have just a low fever, NOT to try to bring the fever down by using Tylenol, Motrin, or cold showers because a fever is simply the body's natural defense against infection, so you shouldn't try to stop it. Any thoughts on this?

Heat causes me severe pain. Pain worse than 5mg Percocet can handle.

Cold actually helps pain.
