Chief Nursing Officer (CNO) of Dicerra, Michael Sandler, relates a clinical safety issue from his past experience. Distraction, fatigue and biases added up in a "Swiss cheese" model that almost resulted in a tragic accident for a patient. Michael also discusses the poor safety reporting mechanism at the time and the importance of analyzing medical errors to prevent or mitigate future accidents.

Michael Sandler is an experience healthcare executive and the current CNO of Dicerra. He is also an Adjunct Professor at the University of British Columbia. He was previously the CEO of NNPBC (Nurses and Nurse Practitioners of British Columbia) as well as a Clinical Practice Educator with Interior Health Authority in Kelowna, BC.
