I remember seeing this on an old Air Crash Investigations episode. Really scary and goes into a lot more detail in the show. The flight attendant's body was never found, lost to the sea. Great show btw.

This happened in Hawaii. The reason for the metal fatigue was because back then maintainance was scheduled based on the amount of flight hours, not take-off/landing cycles (which is what actually stresses the fuselage, as it will pressurize/depressurize each time). Hawaiian routes are usually very short meaning that plane was making a lot more take-offs/landings per flight hour than the designers expected, add in Hawaii's humid and salty environment, and it was really the worst-case scenario. Maintainance guidlines have been revised after this accident.

EDIT: The plane was a Boeing 737-200, it was 19 years old at the time of the accident with 89, 680 flight cycles, but only 35, 496 flight hours on record. That's an average flight time of just 24 minutes.

For those who don’t know this was Aloha Arline’s on Flight 243, it was said that they had maximized the endurance of the panels lining the plane. The decompression over time was wearing out multiple panels in multiple spots which is why a huge chunk of the roof flew off mid-flight, instead of one panel like expected if this were to happened. Only one person died which was a flight attendant who was Clarabelle Lancing also known as CB, her body was never recovered. Fortunately she was the only person to die in this whole incident that could have been much worse. But thanks to the skills of the crew and pilots no other passengers or anymore crew died.
