I had this idea once. Glitcheon.

It was 5:AM. An Eevee with high friendship had an accident where it fell into a box of evolution stones. Eevee's body began mutating horribly as it glowed various colours. The small creature grew in size, new features appearing and disappearing, with no sign of stopping.

The form was a mess of glitches and painfully crawled towards its trainer, as if begging for him to make it stop. After a moment of hesitation, the trainer pulled "Eevee" in and hugged it tightly, patting its head and speaking as calmly as he could.

"I-It's alright...I'm here...I'm n-not afraid."

Slowly, the form stopped mutating. It calmed down and collapsed from exhaustion. When the glowing stopped as well, the trainer tried to scan it, but no such entry existed in the Pokédex.
