To use the needle, you need to be at Dragonlord Placidusax's arena. YOU ALSO NEED TO BE BRANDED BY THE FLAME OF FRENZY. The only purpose of this needle is to get rid of the flame of frenzy, so, if you have the needle and don't have the flame of frenzy, there's no point using this needle.

Yes. This works after the boss is dead.
Yes. This works after the game is completed and the last fight is done
just go to equipment in the arena and use the needle
if you didnt get the grace in the arena, you can go back to the place the same way as you got there

Hmm i already did my frenzied flame ending, then i didn't start ng+, then i used the needle still works, but you can't get different endings anymore since the fractured marika site is gone.
