Can anyone answer the following question, please?

If I have been diagnosed with NMIBC (T-is with carcinoma in-situ cancer), and prefer not go through the long (very long) invasive treatment (e.g. years of BCG, cystoscopy, TURBT, repeated numerous times), and insist to go directly with radical cystectomy (RC) and urinary diversion (e.g. Ileal conduit) option, will Medicare or other private insurance still cover it? In other words, will I be "forced" to go through years of the invasive BCG treatment, before I will be allowed to choose the RC and urinary diversion route?
