How it likely works is it detects ALL cybertronian lifeforms, however he has a way of personally blocking individual cybertronian's signatures such as his autobots, He can also reactivate their signatures again in the case that they ever switch sides.

Sam did his best to warn Prime, Prime was also recently deceived by Sentinel and thrown under the bus several times by Nest operatives. He knew the Decepticons would attack the shuttle in space. Ratchets bio scanners could easily detect the immense pain Sam was in due to the deceptiwatch and informed prime. Agent Simmons possibly would have broached the subject of the autobots being betrayed as he is naturally paranoid as well.

I KNEW IT. 14 years later I always believed Optimus knew Sam was being used as a spy for the Decepticons and he acted in a way that wouldn’t hurt Sam without jeopardizing their counterattack plan
