Connecting two networks together that are in different cities

Sorry, I’m new to networking, and I have a few questions. In this scenario, I believe the two networks between the two cities managed by an organization within one autonomous system right? Does that mean the organization managing these two networks has to run their own cable to have direct private connection between the two cities? What if the organization is located in different part of the country? How will they transfer data between their locations privately? Do they have to route traffic through internet?
