Do you still mill the dark grains the same as you would any other?

Adding flaked barley in the mash, from the start or last 15 minutes?

Would you top mash carapils and say Crystal malt ebc 140 for same time or say 30 minutes instead?

So if the recipe asks for choc malt, carafa etc, I don't add any of them into the 60 minutes mash until the last 15 minutes? For the first 45 minutes just the plain malt like marris Otter for instance?
