1. Patrick shot Sandy but Sandy shoved him into a trombone just before she was murdered, and the screaming was Patrick crying from the realization of the consequences of his actions and/or the sheer pain from the trombone.

2. Sandy attempted to shoot Patrick but missed (or hit Patrick, but because of his invincibility he wasn't harmed) and then shoved Patrick into a trombone as to prevent him from revealing anything about the attempted murder, and the screaming was Patrick being so horribly contorted inside the trombone.
