Pars defect in the lumbosacral region is a pretty common condition. About 6 -8% of the general population has this condition. By and large the people do not experience this condition. By and large people do not experience any problem. A small percent of the population has some problem. These are people who are very active physically like any sports or bending forward and lifting weights, these people experience some difficulty. So when they get a pain like this, with some sort of physical therapy, general correction of the posture, walking, will help them. Whenever the situation goes a little but beyond the pain, that is when there is radiation of pain in to the hip joints, or knee joints or all over the lower limb, that means there is slipping of the bone. The pars defect is called as the spondylolysis, then this can lead to, that is when it is on both the sides, that is the right and the left side, then when both are broken or completely gone, then the bone above can be slipping over the bone below, then the amount of nerves available here reduces here. So the nerve gets compressed. We call this as canal stenosis. This is also called as a secondary canal stenosis because of the reduced space of the nerves. So this can lead to a neurological problem. Mainly it needs neurological assistance. Whenever a patient experiences this condition, put them on physical therapy and put them on exercises, correction of posture. The most crucial exercise here is the abdominal exercise to strengthen the abdomen. The weight reduction has a role and on a long term basis, we have to keep them on regular exercises on a long term basis. If the pain becomes sever, then on a short term basis, we can give them a brace. The brace is to support the back. The commonest brace is the boston brace, but it is used only on a short term basis of a week to 10 days, the rehabilitation takes over. Once the rehabilitation takes over, the muscle strength increases, then you have to take the brace, otherwise it will be an addiction, in the sense you might get dependent on the brace and your life will get not be possible without the brace. Brace is not a treatment, it is only a supportive thing. Exercises, posture correction, weight reduction, building up of you rack muscles and your abdominal muscles is the proper way to treat this. Rarely when there is a radiation, and it gets complicated, then patient may need a surgery also.
