The orange dot is literally just there to tell you that your microphone is active. Edit: Apps have to ask to use the microphone, they can’t just randomly decide to use it. Edit2: Hackers can’t forcibly enable this, iOS is very secure. I made this comment to stop the spread of misinformation.

Context: This orange dot is the indicator that your microphone is being used. If you’re on a call or making a video or talking to Siri or any other activity that uses your mic, then the dot will appear. If it shows up, open the control center and it will tell you what app is using your mic. The same system is used for the camera but with a green dot instead of the orange dot.
