The setup of the trap door is not clear enough

This trap is being used by a Cat Trap Neuter and Release group. On several occasions, with different traps, the trap door and opened automatically resulting in the cat escaping. Great effort is given to trapping and transporting the cat to the vet. This is disappointing to have the cat escape This was particularly upsetting as the cat was released in the parking lot of the vet office. We never found the cat. Why is this happening? Is there a way to tighten the door to prevent this?
