This is close, but wrong. It's played a half step lower.

Same shapes!

So start on fret 5, same shapes, except the a string can ring open.

Chorus starts on a standard c chord, moves to am (sometimes am7) moves to a d then g. Ends on F.

You have it mostly right in terms of shapes! Your position is one fret below where it actually starts. It's the half step dropped tuning getting ya.
