Joker doesn't use real bullets, or even a real gun.
In Persona 5, it's explained that Joker can use a model gun with BBs like a real gun against enemies in game because the enemies believe it's real. And since the enemies believe it's real, they receive real injuries. So it isn't out of the question to assume the smash fighters think his gun/bullets are real too.
