Sam's soul was recently returned to him by Death, one of the Four Horsemen, after being trapped in Hell. To protect Sam, Death placed a "wall" in his mind, blocking out all the memories of his soulless actions and the torture he endured in Hell. If Sam scratches at this wall, it could crumble piece by piece, leaving him shattered. In this episode, Sam and Dean are investigating the disappearance of several women. Sam receives a text with coordinates to Bristol, Rhode Island, so they head there, thinking someone might be in danger. Once they arrive, they discover that Sam had worked a case in this town while he was soulless. Dean tells Sam they need to leave, fearing that the investigation is triggering memories from behind the wall.
