Ischemic Stroke 04: Treatment with tPA

Recombinant tissue plasminogen activator (tPA) is a potentially dangerous drug with a very real risk of intracranial hemorrhage and death (about 7%). So it needs to be used in patients in whom the benefits outweigh these risks. There are two studies (NINDS for 0-3 hours and ECASS III for 3 to 4.5 hours) in which this benefit has been shown. Following these guidelines is essential to give the drug responsibly.

Now I understand that there is controversy within EM about tPA usage. This video does not tread upon that territory. Let's first understand tPA usage & the guidelines. Later we can explore the controversy.

To clarify: NINDS is an organization within the NIH, not the name of a TPA trial. ECASS III is the name of a specific trial.

I didn't get it , when I had MY Stroke, 12/12/2012. Even though I was at the hospital within 25mins,  they were afraid to give it to me (afraid of a malpractice suit is more like it) It cost me a month, in the hospital, years of rehab,  and permanent damage, I still suffer from. Fast fwd. to 2015, touring 49ers new stadium, when my wife, collapsed in my arms, could not speak,  etc..she showed all the signs of stroke, After all that i had been through, all that i had learned.. Instantly, I knew that time was critical, (time lost, is brain lost )and that TP fuckinA were the three letters I would repeat, incessantly, to everyone,   the paramedics, nurses, doctors, people in the waiting room, T-P-A, Tpa, Give her TPA! 40 mins after they administered TPA, She sat up in the ER bed, and, STARTED TALKING! like nothing had happened, no facial droop, no slurring nothing! IT WAS A Miracle! Today still it is a Blessing, I am so thankful that she is ok, so grateful, to the doctors, all of the hospital staff, but still wonder.. (and probably always will ) what if.. they gave it to me, back then? Would I have been as driven to research my case, to educate myself about stroke, would I have been as aggressive about TPA, as I was, when her stroke was unfolding, right before my eyes? If my stroke had not been, so traumatic,  my recovery had not been, so long, painful, frustrating and incomplete, Would I even know, that the three sweetest letters in the Alphabet, Are T. P. A. ?

Ich wurde aus Mach 2018 entlassen, nachdem ich insgesamt neun Monate im Krankenhaus war. Mein Schlaganfall wirkte sich auf meine linke Seite aus, und auch mein Sehvermögen und meine Wahrnehmung waren betroffen. Ich hätte nicht gewusst, wo ich anfangen soll, dies zu schreiben, und es hat eine Weile gedauert, aber ich bin sicher, es ist alles eine gute Übung und wird mir helfen. (Aber ich entschuldige mich für etwaige Fehler! ... Rechtschreibung und Grammatik ... usw.) Sie sagen, Geduld sei eine Tugend, und das mag nach einem Schlaganfall wahrer sein. Ich sehe jedoch jeden Tag Verbesserungen. Bleib stark und rede darüber, wie du dich fühlst. Ich hatte zahlreiche Bereiche meines Gehirns, die betroffen waren, ich werde sehr emotional und weine manchmal. Ich war nicht depressiv, nur ein wenig mit meinem Gehirn "gewachst".
Meine Reise begann im November 2016. Ich hatte mich schon seit einiger Zeit sehr müde gefühlt und mein Mann kam eines Nachts von der Arbeit, um mich sehr schläfrig und nicht sehr aufmerksam zu finden. Er rief einen Krankenwagen. Kurz darauf hatte ich einen Anfall und dann noch vier auf dem Weg! Sie machten eine Blutuntersuchung und stellten fest, dass ich mehrere Blutarmut hatte, mit einem Blutbild von 23, also brauchte ich 4 Einheiten Blut, was sehr schnell erledigt werden musste, sonst hätte ich nicht überlebt. Dies wirkte Wunder für mich und ich war ungefähr eine weitere Woche im Krankenhaus und wurde dann mit dem Plan entlassen, ambulant mehr Forschung zu betreiben. Nachdem ich ungefähr eine Woche zu Hause war, hatte ich einen weiteren Anfall und wurde zurück ins Krankenhaus gebracht. Ich hatte noch ein paar mehr, was zu meinem ersten Treffer am 12. Dezember führte. Ich verbrachte die ganze Weihnachtszeit im Krankenhaus und erholte mich bis zum 31. Dezember gut. Am Silvesterabend hatte ich meinen zweiten Schlaganfall, der einen anderen Bereich meines Gehirns betraf. Dies drehte mir den Rücken zu, aber ich setzte meine Genesung fort. Ich fing an, ziemlich niedergeschlagen zu werden, aber dann erzählte mir mein erster Enkel von Dr. Pejus Kräuterheilmittel, das Schlaganfall geheilt haben soll. Ich war im Krankenhaus, also musste ich mich auf etwas konzentrieren. Nach ungefähr einer Woche zog ich nach Hause. Wir mussten den Kräuterkenner wegen seiner Kräuter kontaktieren. Es lief ein paar Monate lang sehr gut für mich, da ich gerade angefangen hatte, ein paar Schritte zu unternehmen. Zu Hause wurde ich von einem Freiwilligen von Different Strokes besucht, da sie herausfanden, dass ich mich in Monaten nach der Einnahme der Kräuter erholte. waren auch ein Schlaganfallüberlebender. Inspiriere mich! Als ich gut geheilt war, schloss ich mich der Different Strokes 'Familie' (#dsfamily) an. Die Unterstützung durch Kollegen ist fantastisch und jetzt bin ich sogar an der Leitung der Leeds Central-Gruppe beteiligt! Ich bin vollkommen geheilt. so what do you do if someone is having an ischemic stroke but they meet the contraindications for tPA?

My dad is currently in the hospital thanks to this TPA drug...His stroke was on 1/4/18, TPA was given about 1 hour 20 mins from original stroke time. We knew of the risks of hemorrhage but they were slim, so we decided to give it a shot. He was fine the next day and declined the next night. He had a hemorrhage at the cerebellum about the size of a half dollar. Emergency surgery was done and as of today 1/21/18 he's still in the hospital recovering from pneumonia which was caused by his vent tube given at surgery and it was in about 3 days, now he has a trach in his throat. He still has full use of all limbs (moreso his right side than his left, but he can still move the left side some). He has a long recovery ahead of him and i'm just praying there's little to no brain damage. I just wish i had researched more on this drug before hand. It may be a "miracle" drug but right now my father is living this nightmare.

Can you give tpa to a patient who takes plavix? Also no diabetes but prior ischemic stroke with prior tpa administration 10 years ago? Good to give? Also teeth bleeding, whites of the eyes with specs of blood when should we worry.

Great video series Dr. Patwari. Is it advisable to have a radiologist review the CT scans within that 60 minute window, or would you be comfortable ruling out a hemorrhagic stroke in the ER? Another question I have is whether the ASA 160-325mg) can be administered prior to the CT scan. Thank you.

Very good elicitation, but shocked didn't mention full form tPA (Tissue plasminogen activator), Although I am not doctor.
