For those asking where to find this quest: This is from Blood and Wine DLC where you choose the Unseen Elder path and decide not to go to the fairyland. This is the beast that has attacked Orianna's orphanage

Edit: You can go this route and still go to the fairyland to save syanna(aka best ending).Regis asks you one last time to reconsider your decision to visit the Unseen. This way you save the kids and still get the best ending. you can also take the Alpha Garkain trophy which is very unique and rare

This is an Alpha Garkain. The deadliest lesser Vampire you will encounter in the game. Depending on the game difficulty it can either be a fast kill, or a pain in the ass on Death March. He can jump several meters in the air and stun on ground contact, venomous and imune to stun.
