When I was in the military with the only times we would have a friendly fire incident was because somebody completely lost her mind or they were incredibly stupid. In this case, I’m gonna go with the incredibly stupid. The TV is not known for having stupid people I’m guessing this was a DEI hire situation, instead of hiring somebody for their intelligence and abilities.

The selection among these modes is determined by the ship's command team, including the commanding officer and tactical action officer, based on factors such as threat assessment, rules of engagement, operational environment, and system capabilities. For example, in high-threat scenarios, an immediate "shoot" response may be chosen to neutralize dangers swiftly, while in less hostile situations, a "look" or "shoot-look" approach might be preferred to conserve resources and minimize collateral damage.

You would have thought when American ships and aircraft are in a certain space together, they may just consider talking to one another. Plus were told that aircraft emit a signal to prevent this sort of thing happening.
So how much did that Fu## up just cost !!

Never a dull moment during carrier battle group operations. There are a lot of "moving parts". That's not making excuses.
Part of me pities the investigating personnel almost as much as the people involved.
At least there were no fatalities.
