If any American citizen is subpoenaed by Congress to testify and they refuse, the default should be said person is held in jail and all assets frozen (spouse and children have no access to family funds, if applicable) until they comply.

Re the ad at the end, I live in the UK, I can just ring my local doctors surgery for free, get free advice, go for a visit, get examined and if I need any drug or prescriptions that is also free. I can then pick those up for free at a chemist I specify.
