A thin endometrium in an IVF cycle definitely reduces the success of an IVF cycle.
It is seen 1-2.5% of all IVF pregnancy. Dr. Rishma Dhillon Pai- a decorated IVF expect shares her 5 secrets on thin endometrium.
1st secret - Recognize the problem. Unfortunately it is missed many a times. Trans vaginal ultrasound (TVS) along with a hysteroscopy with directed biopsy can rule out endometritis, tuberculosis & ascertain the cause.
2nd secret - Assess the problem with various commercially available tests
• Endometrial receptivity assay for dating (ERA)
• Endometrial function test for dating & pathology
• Test for ruling out inflammation
3rd secret - Hysteroscopy in itself can improve the endometrial receptivity. If combined with endometrial scratching, it may offer respite in some patients, thought the literature is divided on it. Always rule out endometritis by taking a hysteroscopic directed biopsy & treating the infection if found.
4th secret - Treating a thin endometrium has been backed by some studies.
• Intrauterine hCG infusion
• Intrauterine platelet rich plasma (PRP)
• Intrauterine Granulocyte colony stimulatory factor infusion (GCSF)
Primer technique combination of intrauterine infusion of PRP combined with subcutaneous GCSF
