I had an experience like this recently. On Switch Edition, if you're low health and you logout near an enemy, you come back dead. I tried to go to an old save game, but it put me back at home with nothing. I went back to the place I died at (In the Nether, of course) and the path I had made was gone and it mixed two different save games.
