My brother did two tours in Afghanistan. I’m sure he’s been in combat, much of which he never speaks of. What I do know is that is some instances the rules of engagement go out the window when it comes to survival. Soldiers just want to get home and sometimes at any cost. My heart goes out to anyone who has either had to fight in war or was a civilian caught in a war.

One of the big rule you haven't mentioned that is actually followed by pretty much every nation (although not by guerilla or terrorist groups) is uniforms. Every army need to have easely identifiable uniforms and soldiers on duty are not allowed to dress up as civilians or dress up as the enemy. Now mind you, the reason why this is the one rule everyone follows is that everyone agrees that this is just practical. If you dress your soldiers as the enemy for a trap, well, they are gonna do the same thing and soon the whole war will devolve into a free for all with everyone shooting everyone because they can't be certain if they are allies or foes.
