I am facing problems with the Schedule A-Background Declaration (IMM 5669) form. In this form under the mother's details they have asked 'family name at birth' instead of 'family name'. She has never used a family name in her life, neither before marriage nor now, how do I fill her name here?. Please help

Mam no body in my family uses their 'Family name', we only have 'Given names'. How do I fill my and my family's name in all the forms? Instructions for people without a family name are given for only IMM 0008 form, for all others there are no instructions, what do I do?

In the Schedule 3 form, there is a column on the top left side which says "ID/Client Number (if applicable)", what am I suppose to fill here? Can I leave it blank? Will there be any issues if I leave it blank? I am not really sure what to fill in it. I have my UCI number, and it was clearly asked in the IMM 0008 form, but here I don't know what to answer.

I am a student and I have work limit of 20 hrs per week. But this limit is not applicable during the break time and we can work full time. So, basically my question here is that when there was a break I have worked like 36 hrs per week, 25 hrs per week, but otherwise when there was no break and a limit of 20 hrs. I have worked very little like 5 hrs per week or 8 hrs per week, on the very same job. So how do I show it in the IMM 0130 form???
