Thank You! You are a lifesaver, for a nurse like me with dyslexia and right-side deafness. No matter how hard I try to write more and practice it is my biggest struggle.

Love your examples. As an ex ER nurse, I used to chart just like you. Short, precise and to the point. I have worked with many nurses that feel, and have been taught, that they have to write novels in their notes with lots of flowery words. Not necessary. As long as all the details are there, it's good. Thank you. I'm contemplating going back to LTC and the charting can be very time consuming on top of all the other tasks the nurse has on the shift. I'd rather be chatting with my residents.

I just learned from experienced nurses that nursing homes doesn't use side rails for fall prevention. thank you RN Benomi your videos are always helpful.

“Patient was found on floor” not “patient fell” just saying. They could have lowered themselves to the floor. If it was unwitnessed we don’t know and cannot make assumptions in our charting state will comb through.
