People are not listening, he isn't blaming Yamamoto because he let Juha live. It is because he told him when he saw Ishida was alive, that most likely that the quincies were not killed out.
Yamamoto did nothing with that info. If he had acted on it, no doubt they may have found the quincies before they accumulated all their power and forces.
