I was under the impression that it was an expression of the evil he became when he took the serum. Dr Erskin says that the serum enhances what is already inside the person. "Good becomes great. Bad becomes worse."

Uh, I'm pretty sure the movie explained it. If I remember correctly, the doctor said the formula wasn't ready but red skull was impatient and used too early. As such, the unrefined formula had issues that damaged him.

After escaping to the US, he wad able to have the time to finish it properly and work out any problems with it.

I think that Red Skull was not bombarded by the Vita-Rays, since Germany did not have access to Howard Stark’s technology.

The serum also had to be customized to each person individually.
Red Skull just used it straight up without any precautions or adjustments

This was explained in the film. Dr. Erskin tells Steve that The “Super Soldier serum” enhances what already exists in the recipient. Johann Schmidt was a monster at heart and so became one. Steve on the other hand was an inherently good man, a hero at heart and so he became a Superhero. And the guy that was in the Incredible Hulk was at heart an abomination and thus became the Abomination.

Actually in the comics it's because they are 2 different Super Serums. The one Red Skull took wasn't a complete formula so it had unintended side effects. Steve Rodgers took a perfected version.

It's also the same reason why Bruce Banner's powers work the way they do because he also has ties to the super serum, being experimented on as a child in an attempt to recreate the formula.

Both Hulk and Red Skull are failed experiments while Captain America is the only successful super soldier.

I was under the impression that giant metal tube. They spent so long building up was important to the process, and you know helped him not become a deformed monster.

The only reason was that the serum wasn’t when the red skull injected it which led to deformity

I guess I just figured it was merely the classic hero/villain trope from the times...heroes were handsome, and villain were ugly, to tell them apart. Like in the old dicktracy comics.
