Several factors contributed to the incident, including a liquid withdrawal valve that was stuck in the open position, the task being handled by an inexperienced and incompetent worker, improper placement of the propane tank, lack of proper training by Genth volunteer fire staff in handling hazardous materials, and the failure to evacuate the area.
