Having seen a "fish" left inside the abdomen of a patient, I can attest to the need for extensive surgical checklist. Used to keep intestines safe from suturing on the way out, a 'fish" is a bright blue thick flexible oval plastic about 2 feet by one foot. Yes, the surgeons left one behind and as a medical student I pointed that out.
Before that the fish was not included in the surgical checklist for instruments.
