**ANSWER: C. Give Naloxone (Narcan). This is most likely an opioid overdose, a medical emergency. Look for signs like pupil constriction, altered mental status, and reduced breathing. GIVE NARCAN IMMEDIATELY.

(A) A Urine Drug Screen (UDS) is important but can wait. Prioritize life-saving measures.

(B) An MRI Brain is useful, especially if the patient was found on the ground, but it takes time. Administer Narcan first.

(D) Epinephrine is for life-threatening allergic reactions, which would show symptoms like a skin rash, nausea, vomiting, and shock.

This is likely due to fake prescription pills laced with fentanyl, a highly potent drug causing many deaths. Never take pills not prescribed by your doctor.

I’ll break down the life-saving medication, Narcan, in my newsletter this week! Link to sign up is in the description!

These are wonderful teaching posts! Keep them up.

To add a layer of complexity, yes this is the classic appearance of opioid overdose and give the narcan right away! But keep in mind it may not be the only reason the person in unconscious. Remember to always check glucose, assess for trauma, ask friends about other substances. MDMA and cocaine can cause electrolyte derangement and cardiac arrythmias or seizures. Clonidine can cause almost the exact same presentation as opioid overdose.

I think it's
C it can reverse the effects of whatever was taken.

But this one is difficult because we don't know what the patient has taken to be in this state which makes me also think he should have Adrenaline.

I'll trust my gut and go with C

Have you had to help in a code or lead a code even in psych residency?

I’m no doctor, but that sounds like opioid overdose and Naloxone is cheap and otherwise harmless. Maybe harmless. Well, I’m just a Good Samaritan after all… excuse me🏃‍♂️‍➡️

administer naloxone, get Ua drug screen for lab about same time, then send for MRI the rule out head trauma from falling. Ask people that brought him in if the friends know what the drug names are
