I believe this is a force of habit for Dentists. They always talk to their customers/patients like they are kids because they know that pretty much everyone hates of, or is afraid, being there. I am well into my 30s and whenever I go they still speak to me like they did when I was 14. So as soon as any patient sits into that chair, we're children to them. It doesn't help even if I am not even remotely afraid of them any longer, it's just impossible to not wince at the pain whenever it happens due to WHERE it happens even if it is not particularly painful.. just the fact that you lay there and expect it to be. You feel something and then just waiting "Now.. now is gonna be painful. Any second..."
