Sir in pizillo method the patient is pushing head against hands placed on occiput, show it is like neck extension against resistance which show make deep cervical fascia taut anteriorly and make thyroid swelling less prominent?

at 49:25 when we ask the patient to extend the neck against resistance, it becomes more prominent?? Or is it like when we ask the patient to just simply extend it bcm more prominent it due to cervical lordosis.
