Shorted cells... especially if it was left out in the cold, get covered in snow/ice, then thawed out.
Crack open the case and dry/clean it out, I'd bet a dollar it'll be fine.

Easy. Three rows of batteries hooked up in series. Hikoki does this in their tools. Their battery switches from 18V to 36V depending on the tool. By switching I mean the tools are designed to use/combine different pins. Try measuring other pins. It's absolutely possible to make the 12Ah batteries do 18V and 54V. I don't know Milwaukee batteries by their exact details, but I know tool batteries in general quite well.

I had a Milwaukee battery once that smoked every tool I put it into and pulled the trigger on. I bet this battery has the same problem.

Do you have a Low-Z multimeter you can check it with? Perhaps phantom voltage? Also, does it go back to normal if a load is applied?
