I work in a hospital. I was always taught that soap and water is only necessary when there are visible contaminants on the hands, after using the washroom, before and after eating, etc. And to use an alcohol based sanitizer any other times.

I worked for the ambulance, in a hospital and currently in a social psychiatry. Say all say the most efficient way is to first use sanitizer to kill the bacteria and then use soap to wash them all of. But also you're not supposed to soap your hands too often because that damages your skin. Plus you need to use hand cream so your skin doesn't get chapped, bacteria loves dry skin and you can get nasty infections.

I work in the operating room and for operations we use chlorhexidine soap. we also use alcohol based chlorhexidine type sanitizers. these are all hospital grade and reasoning is this kills and inhibits bacterial/viral growth for a lengthy time. regular sanitizers only kill the current bugs on your hands but do not inhibit growth of the bugs that made it through. stay clean everyone!
