The first one is actually caused by mods because it's placeholder for mod's items ( It appears when you place something from mod and then uninstall that mod)

That last one happened to me before, and I had Fargo's active and needed the inferno fork from the dungeon for a certain enchantment, the problem was, the part of the dungeon that spawns the enemies that drop the inferno fork was completely destroyed by the sunken sea, so I had opted out for the Champion of Shadows instead, it was really annoying because the champion didn't drop the enchantment I needed for days
