Why are we counting wide qrs in chest leads and not in the limb ones? Coz in the limb ones it doesn't look wide

1. Rate: normal, tachycardia, bradycardia
2. Rhythm
- R-R interval: regular
- P wave: sinus (II +, aVR -)[if inverted: lead misplaced, dextrocardia, ectopic rhythm],
- P -> QRS: AV association, heart block
- QRS: narrow, wide (VT, )


RSR' in V1-V3, wide QRS: RBBB


RBBB not as ominous as LBBB
RBBB = RSR in V1-V3 + PVC pattern in V5-V6 (ST depression?)

Regular narrow complex: 3 things:
Sinus tachycardia: hypovolemia dehydration, fever, SNS exercise
Atrial Flutter
SVT

Hey, how to you know that you need a V1 lead as a rhythm strip, for all this ? Or is it like anyone can be chosen ?
And how do one decides which to choose ?

What about wandering atrial pacemaker? Shouldn’t you make sure all P waves are shaped the same before saying it’s sinus?
