Thanks for the video! Here we chose to log issue in issue log. But what could be the action to be taken AFTER the issue log is updated ? How to act on it?

Sir in this question, the point that it is a hybrid and then the product owner is also involved and it looks like it is more of an agile so in agile do we document and maintain issue log?

I would recommend that you first display all the 4 options to the viewers. This way we can first pause and think of our answer. Post which you can start explaining each option. This video will not give the viewer time to think about
