Coombs and indirect coombs are not properly defined in this video!! A direct coombs test detects antibodies that attach to the red blood cells in vivo (or within the patient's body), and indirect coombs detects antibodies that are attached to the red blood cells outside of the body or in vitro. When testing to see if the mom's antibodies are attacking the babies cells this is done doing a direct coombs test. To detect if the mom has antibodies in her plasma this is done using an indirect coombs test. These are the two basic tests in the blood bank and can test for all antibodies not just anti-D.

She states that the test depends on where the blood is collected from and this is 100% incorrect. A DAT or Direct Coombs test is run on either cord blood or babies blood (it can also be run on adult blood). The Direct coombs in prenatal testing is looking to see if moms antibodies are attached to babies red blood cells. This test is used in the blood bank to test for other things as well, not just on moms and babies.

A few things I might clarify further, but an EXCELLENT presentation of an often misunderstood issue. I'm a blood banker. We dedicatedly check charts and pharmacy records to make certain Moms get RHIG when required. But admittedly, some of us are frustrated when nurses and doctors are not doing what needs to be done, and are depending on underpaid, frayed peons in the lab give them a worried call. Would love to see a presentation from you on WHY Rhogam or its generic equivalents are crucial to Mom's next child. PS Could you please tell your colleagues we cannot simply hand out blood product to a 600 gram neonate without orders and PROPER identifiers? Blood products are much more dangerous and controlled than any opioid you can name: try waltzing into the pharmacy and asking for a slog of Oxy without patient identifiers! If a caregiver is unsure of hospital protocol, stand strong and DEMAND the training.
