Update: A lot of people are asking how to fill out their name if they only have a given name on their passport. In that case,

1. you can enter your whole name in given name field and for surname, write ‘LNU’.
2. Or you can repeat your whole name in both given name and surname fields.

Both options will not be wrong as People in north india often dont have their last name in the surname field. And you can always clarify that with the visa officer when you’re giving your interview in person. Hope that helps!
