Hi, is it always scored as 4 in the pupil scale? Or is it going to be based on the real patient upon assessment? Also, is it acceptable to put RL in one box in the limb movement if they have the same normal power instead of putting a dot? Thank you

What if the resp is 15, thats a very small box for a 0-20 resp?

In a proper hospital setting with a patient who has suffered an acute head injury surely you're going to have an actual NEWS chart to chart their full vital signs plus a separate neurological observation sheet? This just seems ridiculous and backwards to chart this way.
