He had a congenital disorder that was triggered by the electro-convulsive therapy that Teddy went through as part of his conversion therapy. Basically, Teddy tried to make himself Not Gay and it almost killed him. The fiancé left him at the end because she thinks that Teddy and the roommate/ex-boyfriend still love each other despite Teddy insisting that he doesn't and isn't going to go back to him.

To all those confused about the ridiculous lengths they go to solve cases: the premise of the show is "Sherlock Holmes as a doctor". House is Holmes, and the mystery diseases are the unsolved crimes. It's pulp fiction, not realism.

I felt so sorry for the fiancé. He used her as a prop for what he imagined to be his perfect life. I also felt his ex was really insensitive to her. I get the jealousy aspect, but at least he knew what the truth of what was going on (the lies, not the disease) whereas she was being deceived.
