Most of the radiation deaths would occur within weeks of the attack, and at most, three months afterwards. Those that didn't get a lethal dose of radiation, but got enough radiation to get moderate radiation sickness, would be substantially disabled and unable to work for the rest of their lives. Many would die within a year or two, either by cancer, serious infections, or general debility. Moderate radiation sickness requires specialist medical care, something that will likely not be available in the aftermath of an all-out, general nuclear war.

You are correct in stating that after a massive nuclear war, we'd more or less be thrown back to a pre-industrial state. However, computers and other forms of technology would continue to exist in parts of the world that were unaffected by the war. I doubt that it would take 25 years to even begin to recover from a nuclear war. It would be more likely that 25 years postwar, we'd be back to some semblance of what we knew before the war broke out, but it would be a much more limited world in technological terms. Instead of the internet, we'd probably have fax and teletype machines for long-distance communications.

Perhaps the most realistic assessment of what the aftermath of a general (or even limited) nuclear war could look like, is the British docudrama Threads (1984). It's well worth watching.

Even after nuclear war, we manage to band together after awhile to “be human” but as you mentioned there will be different regimes, politics etc and eventually they’ll start to battle one another. As the terminator said in T2- it’s in our nature to kill one another.

I think that the situation would depend on where you are and what time of the year the attack happened (hopefully not in the winter). I have seen the likely targets for Russian nukes and the closet target to where I live would be is about 45 minutes driving time away, nor is my city of 60, 000 in the dangerous fallout shadow, so we would survive the initial blast and the dangerous fallout fairly well intact. Martial law would be immediately declared and the greater metropolitan area of about 500, 000 (5 small cities, all contiguous) would prepare from an influx of refugees. I don't see order breaking down like in movies or the way this video shows things - in fact, under martial law, things would be tightly controlled. My guess is that we would continue to have electricity for a few days from the nearby coal plant, and indefinitely from the electricity produced by the nearby dam.

I do think the 300 million dead (out of 330 million in the US) after one year is very possible, especially if we have a nuclear winter instead of a milder nuclear fall. In some ways, the lucky ones are those who died instantly in the blast instead of starving later on, depending on how effective the government is in getting food to the survivors, and how quickly clean water and energy can be put into place. I have not seen any official "survive a nuclear war" plans, but my guess is that survivors would be moved and concentrated in various "camps" around the country in warmer areas. This would make food distribution easier and concentrate the medical care, rather than trying to take care of this all around the country. Those who stayed behind might actually face some lawlessness, but the survivor camps would be highly regulated by the military and safe.

The US might end up being a nation of about 30 million, but estimates are that 5 billion or so would die around the world, most from starvation in areas that were not targeted, again, depending on how severe the weather impact was on growing food. I don't see us going back to the "stone age". I hope we never find out.

Might be that a nuclear war unfolds in non nuclear states. For example, Europe becoming the battlefield but not in France, UK, USA and Russia who can respond with nukes if their country gets hit.
