I guess the most common mistake is with noun clauses... a lot of learners say "I don't know where is she". In fact they must ask themselves "I don't know what?". So this is a noun clause and you must just say "I don't know where she is".

8:30 that was my sole problem here. I used to say "....where/what/etc is it" because i thought it must be same as those question sentences. Now im used to use this correct usage.:)
