The rarest is "left the confines of the world while fighting Ender Dragon". The fastest way to do this would require you to use glitches to trick the game into spawning 2 ender dragons, kill 1 of them so the end portal opens and you can take the other through to the overworld, then bring it to the world border which can be done at 8x speed with nether portals but still takes ages, and then get it to hit you while you ender pearl over the world border. Will almost certainly never be done in survival without cheats
