Polycystic ovarian syndrome is a major leading cause of infertility in the reproductive age of women in the present era which is seen around three to seven percent of incidents and it has been seen in the menarche till menopausal age with different manifestation but the most affected age group is 20 to 30 years. The common symptoms which we come across are like oligomenorrhea, irregular menstrual cycles, endometrial hypoplasia, type 2 diabetes, insulin resistance, etc. also sometimes it is associated with hirsutism, acne and other presentations. Need for hour to treat the PCOD is depends on the type of the presentation and also end objective because PCOD is a such a disorder which has various clinical symptoms for example if it is in the case of an adolescent females we aim at regularization of menstrual cycles. If it is in the married woman with the history of unable to conception we aim at the level of regularization of cycles along with the ovulation or aim to resolve the anovulatory cycles or if it is in the late 30s or 40 age with PCOD presentation then we should aim for their metabolic complications. Need for hour for the treatment of PCOD is because of recurrent miscarriages, infertility, anovulatory cycles in the case of reproductive age group of women and also it ends up with insulin resistance type 2 diabetes mellitus, hyperlipidemia and other cardiac metabolic complications. So in this regard PCOD should be aimed for the treatment depending on the type of presentation and manifestation of clinical symptoms. In Ayurveda we do treat PCOD patients with different types of panchakarma treatments like vamana, virechana, nasya and two types of basti. Here depending on the symptoms either we plan single panchakarma treatment or combination of the panchakarma treatments. So why the panchakarma treatments have been administered in the case of PCOD? Because PCOD is a complex pathology involving multiple systems, multifactorial and involving with the multiple functional disturbances where this treatment should be aimed at the root level. Hence panchakarma treatments have been advised and have promising results when treated with these five therapies depending on the presentation. Along with panchakarma treatments later PCOD females have been followed up with different type of shamana chikitsa where the oral medicines have been administered with the help of herbal preparations or herbo mineral preparations till the end objective is achieved. End parameter either you are expecting from regularization of the cycles or ovulation in the case of infertility females that particular parameter should be achieved till then they have been administered with the shamana chikitsa and also finally they are end up with the rasayana chikitsa aiming at the level of nourishment of all the dhatus because PCOD involves different levels of the tissues.
